The “Safe Access Oahu” program is geared up to take effect on Monday, September 13, 2021. Employees must show proof of full vaccination or a negative COVID-19 test result each week in order to operate and customers must show full proof of vaccination or a genitive COVID-19 test result taken within the previous 48 hours in order to enter the establishment.
If you are one of the 400 members that got your Johnson and Johnson vaccination at the District Hall on April 3, 2021, and are trying to create your digital hawaii smart health card, here’s what you need to know:
1. Take a picture of your vaccination card with your phone.
2. On your phone or computer desktop, go to Safe Travels here and create a profile if you haven’t created one already.
3. Once you create your profile, click on the “Hawaii SMART Health Card” picture
4. There will be a pop up with instructions on how to obtain your Digital Hawaii Smart Health card. Scroll down until you see “Create a Hawaii SMART Health Card” in blue and click on it.
5. There will be another pop up called “Document Details”. This is where you will fill out your information.
a. Type of Vaccine – Johnson and Johnson
b. Date of Dose – 4/3/21
c. Lot Number – The Lot # is located on the front of your vaccine card, on the sticker. It should be 7 numbers long ending in *609
d. Upload the picture of your vaccine card
e. Further down, you will be asked to provide the
1. Name of Provider – Hawaii Pacific Health
2. Location – Local 3, 2181 Lauwiliwili St., Kapolei, HI 96707
6. After signing and accepting your signature, click “submit”. In a few moments, your name should pop up under “Verified Vaccination Documents” along with a QR code that you can use to show establishments that require it.
